Pearl Sprites (known in Japan as Pearl Keepers) are gray haired fairies in green dresses that live inside giant red flowers. When they first appear only the closed red flower is seen. But whenever Starfy or Starly attack them they'll emerge from their flower to give them pearls and big pearls. The player can do this two times but if done a third time the Pearl Sprite will be defeated like any other enemy. A Pearl Sprite also appears in Big Bossdown where she appears after every boss battle to restore Starfy or Starly's health. In the Game Boy Advance games and Densetsu no Starfy 4 they may also throw spikes other than pearls.

In the Game Boy Advance games, there are enemies known as Mandogora which resemble a green Pearl Sprite when they are closed up. Both Pearl Sprites and Mandogora are Rare Enemies in the Game Boy Advance series. Pearl Sprites are also a Rare Enemy in Densetsu no Starfy 4.

After beating the game, if Starfy meets Himandodo in the Sea of Sky level and clears his minigame Toretore Fruit on hard mode, he will lead Starfy to a room with a Pearl Sprite. The way they give pearls, big pearls, and spikes to Starfy is random.

After beating the game, a Pearl Sprite will appear in the first level of Large Tree's Forest.

Location in Densetsu no Starfy 3

A Pearl Sprite will appear in the third level of the Jungle world right under the Goal.

Location in Densetsu no Starfy 4

After beating the game and getting past the Tobira Majin in the third level of Degil's Castle, a room full of six Pearl Sprites will appear. In this game, the pearls will be thrown faster and there will be less spikes.
